Thank you for your interest in becoming a Rotary Authorized Service Provider which encompasses the Rotary, Forward Lift,  Direct Lift and Chief Collision Technology brands.   

Rotary Authorized Service providers represent the industry’s best support network and, as such, carry strict affiliation requirements.

There are four basic requirements to becoming a Rotary Authorized Service Provider:

1. You must be an established company with a W-9 (country equivalent if outside of US)
2. Company must carry a minimum of $1,000,000 USD General Liability Insurance
3. Company must carry a minimum of $1,000,00 USD Products/Completed Operations Liability Insurance
4. Company must carry Worker’s Compensation Insurance
